Agrace is a nonprofit, community-based health care organization committed to providing compassionate hospice and palliative care across southern Wisconsin. Since its founding in 1978 by a group of dedicated volunteers, Agrace has grown into the state’s largest nonprofit hospice provider, serving individuals and families coping with serious or life-limiting illnesses. With offices in Madison, Janesville, Baraboo, Dodgeville, Watertown, and Wauwatosa, Agrace delivers personalized care in private homes, skilled nursing facilities, assisted living communities, and other residential settings ensuring that each patient’s care is tailored to their unique needs and circumstances.

Guided by a mission to offer personalized care and support for life’s changing health needs, Agrace strives to create a future where everyone receives extraordinary care and guidance during times of serious illness or aging. As a proud member of the National Partnership for Healthcare and Hospice Innovation (NPHI), Agrace works alongside other nonprofit organizations nationwide to share best practices and continuously improve care quality. For more than 45 years, Agrace has been a trusted leader in both caregiving and education around end-of-life issues, always upholding a culture rooted in compassion, dignity, and respect for those it serves.
