Hospice of Montezuma
Hospice of Montezuma is a nonprofit organization dedicated to delivering compassionate, interdisciplinary end-of-life care to individuals and families across Montezuma, Dolores, and parts of San Miguel and San Juan counties in Colorado. Established in 1985 and formally incorporated in 1988, we are committed to enhancing the quality of life for those facing life-limiting illnesses by providing comfort, dignity, and support throughout their journey. Our team-based approach addresses the physical, emotional, and spiritual needs of both patients and their families, with care delivered in homes, assisted living communities, nursing homes, hospitals, or wherever patients feel most at peace.
Beyond hospice and palliative care, Hospice of Montezuma offers comprehensive support services including caregiver education, grief counseling, and bereavement support for a minimum of 13 months following a loss. Our services are provided at no cost to the community and are supported by a dedicated team of professionals and volunteers. We are proud recipients of the Hospice CAHPS Honors Award, reflecting our continued dedication to exceptional patient and caregiver experiences. Through collaborations with programs like Pet Peace of Mind, we also ensure patients can maintain the bond with their beloved pets. At our core, we believe every individual deserves to face the end of life with dignity, surrounded by compassion and understanding.
